as much as i hate supportin complains, as am a medical staff myself, it was rather a unforgivable error which should not happen at the first place. anyway, it was fatal. so in the end the family was not happy things were handle as no official feedback was given at all. (usual story) all the family wanted was an explanation of how the error was done and probably an apology at least. easy way is to go to press, or take legal action but at the moment, trying out the less damaging way first. .. so the best way is to lodge an official complain.
